| R.I. hospital reprimanded for wrong-site surgery |

Providence, R.I. 10/13/2008 - An investigation into a wrong-site knee surgery at 221-bed Miriam Hospital, Providence, R.I., has resulted in a state health department reprimand and a signed consent agreement by the hospital to correct the procedural problems that led to the wrong-site surgery, according to a Rhode Island Health Department news release.

The agreement calls for Miriam to fix problems related to its pre-procedure review of patients’ surgery details; change the pens used to mark surgical sites to ensure the markings cannot be washed off prior to surgery; hire a consultant to evaluate the hospital’s policies and protocols for compliance with patient-safety and clinical-outcomes best practices; and design and implement a near-miss reporting program among other requirements. The wrong-site surgery incident occurred and was reported by the hospital on Sept. 19. In addition to the state health department investigation, a CMS review of the incident also found that hospital staff failed to follow required pre-surgical checks implement to prevent such mistakes, according to the news release. -- by Shawn Rhea What do you think?
